The honest distance
Malpensa is the long way to Lake Garda. It is about 160 km and roughly 2 hours by road to Desenzano, and 2 to 3 hours to the shores further up. Compare that with Verona at about 32 km and 28 minutes to Sirmione and Bergamo at about 79 km and about 50 minutes to Desenzano, and the picture is clear: fly into Malpensa when the fare or the long-haul routing makes it the right airport, not because it is convenient for the lake.
That is worth saying before you book flights rather than after.
Train or car
The train is a genuine option here in a way it is not from the other two airports. Malpensa to Desenzano takes about 2.5 hours via Milano Centrale, which is close to the driving time.
What the train asks of you is a change in Milan with your luggage, and something at the other end unless your hotel is beside a lake station. What a car gives you is one vehicle, door to door, that leaves when your flight actually lands rather than when the timetable says. Two people with cabin bags going to Desenzano should take the train. A family of five going to a villa above the eastern shore should not.
How the car is booked

Child seats on request is the category norm for Italian private hire, and on a two-hour run they are not optional to think about — send ages and heights with the flight number. Tell us the number of large cases too, because that decides the vehicle. Stops on the way are usually possible for an agreed fee, and on this route they are a sensible request rather than a nuisance.
Season and cancellation
The lake’s season is April to October; summer Saturdays are the heaviest on both the Milan ring roads and the lake approaches, so an early or late slot beats a midday one. Cancellation terms are set by each company and come with your confirmation, against a lake-wide norm of free cancellation 24 to 48 hours ahead on booked services.
