The route, honestly
Porto to Pinhão is about 2 hours door to door by road. The first stretch is motorway and unremarkable; the last part, the N222 from Peso da Régua, is roughly 25 to 30 minutes of switchbacks above the river and the reason to arrive in daylight if you can choose. Régua itself is about 1.5 hours from Porto, which is worth knowing if you are weighing a stop on the way.
Train or car — the real comparison
This is the question most people actually have, so here is the straight answer. The Linha do Douro train from Porto São Bento to Pinhão takes about 2 hours 25 minutes, and only two direct regional services run from São Bento each day, at 09:25 and 13:25; anything else involves a change. The ride along the river is one of Europe’s good train journeys, and if you are travelling light and your day fits those departures, take it.
A private car wins when the timings do not line up: an arrival flight, a late checkout, three suitcases, small children, or a hotel that is not in Pinhão itself but up in the terraces. It is also door to door, which the train is not — and it lets you break the drive at a quinta or a viewpoint instead of watching them go past a window.

Arriving in Pinhão
Pinhão is small and unusually well arranged for people without a car. Quinta do Bomfim and Quinta de la Rosa are walkable from the station and the pier, and the town has two rabelo piers — Cais A on Travessa da Marginal, with free parking about a minute away, and Rua da Praia. Shared river runs from those piers are about 50 minutes to Romaneira or about 1 hour 50 to Tua, from around 15 EUR and 30 EUR per person.
Timing, season and cancellation
Core season is May to October. In August the valley can reach 40 C, hotter than the coast you are leaving, so an early or late pickup beats a midday one. Harvest, roughly 10 September to 5 October and peaking in the second half of the month, is when cars and rooms in Pinhão go first — book the transfer when you book the hotel.
